Each year, airports and state bodies handle many occurrence notifications. As an independent investigator, I work with all teams involved to establish what happened, why — and what needs to change to prevent it from happening again.

How do I investigate
Each year, airports and state bodies receive many occurrence notifications from their local partners. As an independent investigator, I request input from all teams and airport stakeholders to establish:
- what happened
- how it happened
- who was involved
- why it happened
- where it happened
- what needs to be done to prevent recurrence
Every detail matters — no two occurrences share the same circumstances.

Field investigations – if on-site visit is required
This part of an investigation takes place in order to verify environmental impact that might have been involved in particular occurence. Runway characteristics, surroundings and aerodrome resource management and processes are being verified and analyzed at this stage.
Detailed enquiries
Investigations involve stakeholder inquiries and on-site workshops to reconstruct the sequence of events and understand the circumstances. Throughout this process, safety recommendations are issued to address identified risks and prevent future recurrence.
Consultation
When all evidences have been analyzed, conclusions drawn, a draft report goes through several stages of internal review. We are then required to issue a confidential draft report to those stakeholders that have been involved in the investigation.

All occurrence investigations are conducted in acccordance with the following regulations:
- Latest revision of ICAO Annex 13.
- Regulation (EU) No 996/2010 of the European Parliament and of the Council of 20 October 2010 on the investigation and prevention of accidents and incidents in civil aviation;
- Regulation (EU) No 376/2014 of the European Parliament and of the Council of 3 April 2014 on the reporting, analysis and follow-up of occurrences in civil aviation;
- Regulation (EU) 2015/1018 of 29 June 2015 laying down a list classifying occurrences in civil aviation to be mandatorily reported;
- Regulation (EU) 2021/2082 laying down the arrangements for the implementation of Regulation (EU) No 376/2014 of the European Parliament and of the Council as regards the common European risk classification scheme.
Each and every single investigation is always carried out based on Just Culture policy.
